If you need to make an appointment please visit the website. There is a lot of information there that may help you and in fact remove the need for the appointment.

If you are unable to access the website, you can ring the practice and our reception staff will complete the form with you over the phone.

GP Appointment

If you feel you may need a GP appointment visit our Consulting Room and complete the ‘Get Help for Any health problem’ form. Please complete this to the best of your ability as it will help our medical team assess your problem more efficiently.

Our medical team consists of GPs, our practice paramedic and our advance nurse practitioner.

We will respond to your request on the day you submit it, and contact you either by e mail, a telephone consultation, a video consultation or arrange for a face to face appointment. This will be on the same day as you submit your request so please ensure you are able to undertake a consultation on that day.

If you want to speak or see a specific person please contact the practice on a day when they are working. This is shown on the “Consulting Days” page which gives you the days in which a specific person is working.

Some specialised GP appointments such as minor surgery and contraceptive procedures will be booked in advance.

Practice Nursing Team

If you require a nurse appointment please completes the ‘Request an appointment with a Nurse or Health Care Assistant form which is found in the Consulting Room.

Please provide as much information as possible, including your availability to attend for an appointment. If appropriate we can book your appointment and inform you via e mail. You just need to let us know if it is not suitable.

Home Visits

All requests for home visits are assessed by the Duty GP.They are only for those who are housebound or too unwell to attend the practice. Please make the request via our Consulting Room and complete the ‘Get Help for Any health problem’ form or by phone if you are unable to do this. Please make the request as early as possible, before 10:30. The visit may be undertaken by our Paramedic Practitioner, by a Paramedic Practitioner covering all Woking Practices or in specific cases by a Southview Medical Practice GP.

Home visits are also undertaken by the District Nursing team for those who are housebound or too unwell to attend the practice. Referral to the District Nursing team is by another medical professional and visits are assessed by the team prior to undertaking any visits. We do not have any access to their appointment log. If you cannot attend an appointment for any reason please call inform us online as soon as possible in order for us to give the slot to someone else. If you cannot access the website please call us.

Other Appointments

NHS GP LIVI Video Consultations

Through the NICS GP Federation you can have NHS GP video consultations via the LIVI App.

Appointments are available Monday – Friday 08:00-20:00 and Saturday – Sunday 09:00-12:00.

It is available for all over 2 years of age.

Consultations are with a GP who has access to your medical records and records the consultation in your GP notes.

Extended Access Initiative

The NICS GP Federation has been providing Extended Access Appointments with GPs, Nurses, HCA and Nurse Practitioners outside normal Surgery Hours within the Woking area. This has temporarily been suspended due to Covid but we hope it will resume later in the autumn.

First Contact Physio

As part of the Extended Access Initiative provided by the NICS GP Federation there is a direct access physiotherapy service. This is suitable for musculoskeletal problems such as back pain, knee pain, sporting injuries. To book this please contact reception via ‘Ask reception a question’ asking for a Physio First Appointment, please include your availability and a brief description of the condition you wish to see the physio about.
